To ensure patient confidentiality and compliance with data protection requirements, individuals collecting documents containing patient-identifiable information must present an acceptable form of photographic identification at the time of collection to show the receptionist.
To protect your confidential medical information, we kindly ask all patients to present valid identification when requesting or collecting:
- Medical letters
- Test results
- Copies of records
- Any documents containing personal health information

Accepted ID:
- Passport
- Driving Licence
- National ID Card
- NHS App with your name and photo
- Utility Bill i.e. Council Tax bill

This policy is in place to safeguard your privacy and ensure your data is only shared with you or your authorised representative.
If someone is collecting information on your behalf, they must bring:
- A signed consent letter from you (we may call you to confirm the information is correct)
- Their own and your valid ID
If you would like a representative to be added to your record, please collect a consent form from the reception desk.
If you do not have any of the above ID, please speak to a member of management.
