If you are happy with something the surgery has done or is doing or if you have concerns about something then please let us know!  

The practice strives to ensure that our level of care and customer service is of the highest standard at all times and if we fail to deliver this we want to know so we can look to see why and take action to correct it.

Please pass any comments on for the Practice Manager or you can write a comment in our comments book which is at the end of the reception desk.

With patient's help the doctors and staff can ensure that The Boulevard offers the best service for patient's possible.

All patients are welcome to get involved with our patient group and attend the group meetings which will be advertised on here and in the practice.

We have the NHS Friends and Family test now in place - you can complete a short form in the surgery or complete the survey on either the link below or the link on this website.

We will be opening a consultation shortly on the viability of and options regarding services at the Horne St site, please keep an eye out for that on the website and respond.
